Release and Waiver (Read Carefully)

Assumption of Risk:

I understand that participation in rhythmic gymnastics, fitness, training, camps, classes, performances, and related activities involves inherent risks. These risks may include, but are not limited to, falls, collisions, equipment-related injury, cuts, bruises, sprains, fractures, paralysis, and in rare cases, death.

Participation:

I give permission for the student named above to participate in all activities offered by RRT and ISA, including the use of training equipment and facilities.

Release of Liability:

To the fullest extent permitted by law, I release and forever discharge Rhythmic Ribbon of Texas (RRT), International Sports Academy (ISA), and their owners, directors, coaches, staff, volunteers, agents, and facility providers from any and all claims, liabilities, demands, actions, damages, or causes of action arising from or related to participation, whether occurring before, during, or after activities.

Indemnification:

I agree to indemnify and hold harmless RRT/ISA from any claims or expenses (including reasonable attorney fees) arising from the student's participation, including claims brought by or on behalf of the student.

This waiver is intended to be broad and inclusive. If any portion is found unenforceable, the remaining portions shall remain in effect.

Medical Consent

I authorize RRT/ISA staff to administer basic first aid and, if needed, to obtain emergency medical services for the student. I understand that RRT/ISA does not provide medical insurance and I accept responsibility for all costs associated with medical treatment.

Media Release (Choose One)

Photos and videos may be taken during classes or events. We will not associate a child's name with their image without parental permission.

Policies (Initial Each)
Annual Membership Fee (camp excluded): A $90 annual membership fee is due at registration. If you are a current member and your membership is not due, you will not be charged. This fee renews every 12 months for active students.
Payment Information: Tuition is due each month on the 26th, no later than the 1st. Late fees: $25 after the 5th, $50 after the 10th, and $75 after the 20th. Returned check fee: $30. Participation may be suspended if the account is not current. PayPal is accepted with a $15 processing fee. PayPal email: rhythmicribbonoftexas@gmail.com. Online payment may also be available via QuickBooks.
Cancellation Policy: To avoid charges for the next month, a drop class form must be submitted at least 30 days prior (beginner and recreational classes) and 60 days prior (competitive team and adult fitness). Drop forms available by email: rhythmicribbonoftexas@gmail.com.
Make-Up Classes: Contact Mrs. Rasin to schedule a make-up class for beginner classes. No make-up classes are offered for competitive program athletes.
Closures: No classes on major holidays: January 1, Memorial Day, July 4 week, Labor Day, Thanksgiving (Thu-Fri), Christmas Eve, Christmas Day, New Year's Eve, and during inclement weather as determined by RRT/ISA.
Pick-Up Policy: Students must be picked up inside the building promptly after class. Staff is not responsible for unattended children before or after class times unless prior arrangements have been made. If a child is not picked up within 30 minutes after closure and we cannot reach a parent/guardian, staff may contact law enforcement to help ensure safe transport.
